So how does it work exactly? After you book your first appointment with us, you will be emailed your intake form, which will include a section where you can upload or take a photo of your benefits card. Complete your intake form, and that’s it! We we will review your info at your first appointment. We can even run an eligibility check for you to see if you’re covered for the service and how much is covered for you. Just ask us! We are happy to help.
