We offer direct billing to 40 Insurance companies! Our list is always growing as we are always striving to better serve you. If you do not see your insurance company listed below, please call us or leave us a note on your appointment booking and tell us their name, and we would be happy to help you out.

Primary and Secondary Insurance

It’s so awesome to have coverage from two insurance companies, but it can also turn into a bit of a headache on your end having to submit to both of them. We would LOVE to submit to both of your insurance companies, but at this time, the portals that we use do not allow billing to the secondary insurance companies. We know, we really wish we could do this for you. We have found over the years that we can submit to the secondary insurance company if it is Greenshield - but only Greenshield. It is the only insurance portal that we’ve encountered that will ask us how much is covered by the primary insurance company.

But you both have Canada Life? Even if you have Canada Life for both your primary and secondary insurance company, and it would make sense there were would a co-ordination of benefits, we still cannot submit to the secondary insurance policy - the Telus eClaims portal that we use does not allow include a field for us to tell them how much was covered by the primary policy. We are sorry we can’t, but the good news is you can submit your receipts for reimbursement directly on your member site.

OTIP RAEO - Ontario Teacher’s Insurance Plan

We can submit claims for OTIP RAEO (Ontario Teacher's Insurance Plan), which is through Manulife Financial, so select Manulife on your insurance form.

Intake Form

As soon as you book an appointment with us, you will receive the intake form for that particular service, which includes the following sections: personal contact info, health history, credit card, insurance, and consents. Please complete this as soon as you can. We require it to be complete more than 48 hours before your appointment start time.

If the appointment is for your child, but the appointment is in your name, you can call us or send us a quick email and let us know your child full name and date of birth, we we can create a profile for your child in Jane, and then connect it to your parent profile.

On the intake form there is a section were you can upload a photo or screenshot of your extended health insurance plan card. We need the following information for us to submit to your primary insurance company.

• Select the insurance company name from the drop-list

• Member ID or Employee ID

• Policy Number or Contract Number, Group, or Plan (a few different names for this)

• The Plan Member’s Name

• The Plan Member’s Date of Birth

Please insure this information is accurate and available to us by the time of your visit. If we cannot submit to your insurance company, we will charge the credit card on file and can submit to your insurance company for your next visit. If you have any concerns or questions when completing your intake form, please call or email us to ensure we have the correct information. We will make a couple of attempts to submit your insurance, but if we cannot submit it due to errors in numbers, missing date of birth, and lack of completed information, then we will need to charge your credit card on file and send you the receipt.
